Premier Home Accessories wants to make it as easy as possible for you to order. Although we are not able to offer the convenience of immediate credit card acceptance on-site, we plan to make it available in the future. 1. Print this order form, complete it, and mail it with your check, money order, or credit card information to our address listed on our Contact Us page;

2. Call 1-859-351-4750 during regular business hours, and have your credit card information handy (we'll process you right over the phone); or

3. Email us the following information: your name, your shipping address, the item number, the price each, and the quantity you want (you will receive notification from ProPay that payment is requested, and you will be sent to a secure payment site to enter your credit card information).
